What is DPC?
Direct Primary Care, often called DPC, is a membership-based approach to primary care.
Instead of billing Revive visits through insurance, members pay an affordable, predictable monthly membership fee directly to the practice.
This model allows Revive to focus on something that traditional healthcare often struggles to provide - TIME.
Time.
Time to listen. Time to explain.
Time to think. Time to follow up.
Time to build a relationship.
And because Revive does not bill insurance for your DPC care, the experience can stay centered on the person sitting in the room rather than the billing code attached to the visit.
Who is Revive DPC for?

Revive DPC is available for adults age 18 and older.
Your Membership
First month — $149
Your first month includes a Comprehensive Initial Assessment, giving us the opportunity to understand your health history, current concerns, goals, medications, lifestyle, preventive-care needs, and priorities moving forward.
Beginning month two — $79/month
Your membership continues month-to-month and provides ongoing access to the primary-care services included within the Revive DPC model. No long-term contract.
What about insurance?
Revive does not bill insurance for your DPC membership or DPC visits.
That does not mean you shouldn’t have health insurance. Health insurance remains important for services outside the scope of DPC, such as:

Certain laboratory testing

Other services provided outside Revive
Your DPC membership and your health insurance serve different purposes.
DPC is not health insurance
and is not a substitute
for health insurance.
Primary care is only part of the story.
At Revive, primary care is grounded in a whole-person approach.
We may talk about blood pressure and medications.
However, health rarely exists in isolated pieces. So.... we also talk about:
Sleep. Nutrition. Movement. Stress. Strength. Metabolic health. Prevention. Recovery. Goals. Life.
Our role is to understand your goals, where you are no in your journey, and then partner with you for success.

Meet Revive's founder and provider.
Hi friends, thank you for taking time to visit our site and learn more about Direct Primary Care. After years of caring for people across traditional healthcare settings, I knew there had to be a better way. I wanted to create a space where there was room for something that often gets lost in modern healthcare —conversation.
I founded Revive to provide that space. Our goal is not to simply treat a diagnosis. It is to understand the person carrying it. It's a thoughtful and blended approach that includes primary care, prevention, lifestyle and metabolic health, education, and collaborative decision making. We believe this is essential to helping patients better understand their health and take meaningful steps forward.
I would love to share more with you about the Revive and why we exist, share more information about Direct Primary Care, and talk about whether this is a good fit for you and your wellness goals.

DPC FAQ
How much does membership cost? The first month is $149 and includes your Comprehensive Initial Assessment. Membership is $79 per month beginning with month two.
Is there a long-term commitment? No. After the first two months, membership continues on a month-to-month basis. We do request a 30-day cancellation notice.
Does Revive accept insurance? Revive does not bill insurance for any services, including direct primary care.
Should I still have health insurance? Yes. DPC is not health insurance and does not replace coverage for emergencies, hospital care, specialists, advanced testing, and other services outside the DPC model.
Can Revive prescribe medications? Yes. Medications may be prescribed when clinically appropriate and are sent to your preferred pharmacy. Your pharmacy typically bills your insurance under your prescription drug plan. If you have specific questions, please speak with your pharmacy directly to receive the best guidance.
Are laboratory tests included? Some in-office testing may be available as part of your care. Routine annual labs including complete blood count, metabolic panel, and lipid panel are included as appropriate. When additional outside laboratory testing is needed, we’ll discuss available options and associated costs before moving forward whenever possible. Typically, the laboratory bills your insurance company directly for those lab services.
Do you offer vaccines? Revive does not currently provide vaccines.
Can I use an HSA? We are encouraged with the new 2026 coverage guidelines related to the use of HSA dollars to for DPC membership. However, plans differ and you are encouraged to discuss your specific plan benefits with your benefits administrator.
Who can join? Revive DPC currently serves adults age 18 and older. Additionally, due to government regulations, we are not able to provide services to Medicare/Medicare Advantage covered individuals at this time.
